Further to running a successful soft furnishing business for over 12 years, I have spent several years supporting vulnerable adults and individuals with complex health and social care needs, in a variety of roles within different organisations, services and boroughs and have gained an extensive understanding and knowledge of what patients require, to ensure their journey through the health and social care systems is as streamline, holistic and person centred as possible.
I have extensive experience of integrated working across all sectors, with colleagues from Primary, Secondary and Third Sector services, both statutory and non-statutory, within both the hospital and community settings.
I was involved in the high profile Greenwich Co-ordinated Care Test and Learn Pilot, from its inception in 2014 and I currently manage the Age UK Bromley & Greenwich Care Navigation Service, via a Frailty Pathway, based at the Elderly Integrated Care Unit at Orpington Hospital, in Bromley.
I remain very passionate about integrated working and delivering high standard, personalised patient care.
Through my own personal lived experience, (father who had Alzheimer’s, mother who had palliative care needs and a cousin with learning disabilities), I also have a comprehensive understanding of patient care needs/expectations, as well as the challenges they encounter, particularly as someone with Frailty, when navigating though the health and social care system.
As PPI representative on the NHS E/I Frailty Network Clinical Leadership Group and Co-Chair of the PPI sub Group of the network, I continue to advocate to ensure that patient voices influence strategies, agendas and Frailty service delivery and that they are involved and empowered to make informed decisions about their health care and well-being.
